Thursday, August 15 2:00 p.m.-7:00 p.m.
Tuesday, August 27 9:00 a.m.- 3:00 p.m.
Tuesday, September 24 9:00 a.m.- 3:00 p.m.
Tuesday, October 22 9:00 a.m.- 3:00 p.m.

The Adena Mobile Clinic consent form must be signed by a parent/guardian before a student can be treated by Adena Health staff. The consent form is part of the required forms to be completed in Final Forms.
